진단(국시책)

안압 21mmhg이상

CD ratio cup/disk ratio 0.6이상

두눈 CD ratio 차이 0.2 이상

양측 안압차이 5mmhg

폐쇄각 녹내장

방수의 생성과 유출의 균형이 무너져서 발생함

• Conjunctival redness
• Corneal edema or cloudiness
• A shallow anterior chamber
• A mid-dilated pupil (4 to 6 mm) that reacts poorly to light

증상

· Decreased vision

· Halos around lights

· Headache

· Severe eye pain

· Nausea and vomiting

진단

• Visual acuity
• Evaluation of the pupils
• Intraocular pressure
• Slit lamp examination of the anterior segments
• Visual field testing (either by confrontation [finger testing] or by formal methods, depending on the acuteness of the clinical situation)
• Gonioscopy (see 'Gonioscopy' below)
• Undilated fundus examination

급성 폐쇄각 녹내장

1시간내에 안과에 보내라

그렇지 못할때는 응급 처치하라

아래의 regimen을 분당1회 투여

· 0.5% timolol maleate (Timoptic) nonselective beta-blocker , Blocks both beta₁- and beta₂-adrenergic receptors, reduces intraocular pressure by reducing aqueous humor production or possibly outflow

· 1% apraclonidine (Iopidine), and potent alpha-adrenergic agent , 방수의 생성을 줄임

· 2% pilocarpine (Isopto Carpine) Directly stimulates cholinergic receptors in the eye causing miosis , loss of accommodation (by constriction of ciliary muscle)

필로칼핀 Mechanism of Action (축동, 섬모체근육 수축)

Directly stimulates cholinergic receptors in the eye causing miosis (by contraction of the iris sphincter), loss of accommodation (by constriction of ciliary muscle), and lowering of intraocular pressure (with decreased resistance to aqueous humor outflow)

섬모체 근육이 수축할때 trabecular meshwork를 열어서 방수 유출을 돕게 된다

oral or IV acetazolamide, IV mannitol, or oral glycerol or isosorbide를 자주줄수 있다.

uptodate추천 250mg oral acetazolamide

· Carbonic Anhydrase Inhibitor 일종의 diuretics(방수의 생성을 줄이는데 쓰는)

다른 약은 안과에서 써라. 왜나하면 angle closure가 확인이 되어야 한다.

Attack이 지나가면 laser iridotomy를 해야함. 그것도 잘안되면, peripheral iridectomy

개방각 녹내장

치료시작 안압: consensus는 없다.

case by case

IOP 22 to 24 mmHg 이면서 optic nerve 괜찮고, visual field 괜찮으면 치료 안할수 있고

IOP 18mmhg 이면서 visual field 나쁘고, cupping이 관찰되면 치료한다.

목표 안압 : IOP of ≥25 to 30 percent below initial IOP

치료

보통 몇가지 약제를 투여하게 된다.

하루 3~4회 투여하게 된다.

BB : 방수 생성 억제

topical prostaglandins(포도막 공막 유출을 증가시켜 안압감소)

as initial therapy로 추천

effective and well tolerated, with few systemic side effects

latanoprost 이 hyperemia 부작용이 적다

Alpha adrenergic agonists

beta blockers와 비슷한 효과

눈 부작용 많다 : allergic conjunctivitis, hyperemia, and ocular pruritis

Systemic carbonic anhydrase inhibitors

새로운 topical에 많이 대치되었다.

Topical carbonic anhydrase inhibitors는 다른 먹는 약물보다 효과가 떨어진다.

Cholinergic agonists

beta-blocker보다 전신적인 부작용 적다

small pupils, myopia, and increased subjective visual disturbance 같은 부작용때문에 인기가 줄었다.
